The educational institution responsible for the student can withdraw a student from a course.
BYU IS must receive a Withdrawal Request Form from an authorized individual.
Once completed, email, fax, or mail this form.
- Email by clicking on the “submit by email” button on the form
- Fax the form to 801-812-8208.
- Mail the form to:
BYU Independent Study

Provo, UT 84602-0300
Your request will be processed, with the refund based on the date we receive your request.
If the withdrawal is processed within 14 days of enrollment – Full Refund
If the withdrawal is processed 15 to 90 days after enrollment – refund credited to your account minus $30 student management fee.
If the withdrawal is processed 91 or more days after enrollment – No Refund
Once a final exam has been requested the course cannot be withdrawn.

RELA-327-200: The Pearl of Great Price (Online)
I have found that I will not be able to finish this class at this time due to my entering the mission field. I will not be able to take the final while in the field and will not be able to extend the course sufficiently to finish upon my return. It is for these reasons I see no other option than to withdraw from this class for the time being.
Algebra 2, Part 2
Unfortunately I will have to withdraw from 'Algebra 2, Part 2' course due time purposes. Thank you for understanding.
